Call the company as quickly as possible to notify them of the death so they will not draft the next premium. The company will email, fax, or mail the claim forms to the beneficiaries. Be prepared to have the date of death, cause of death, and current beneficiary address and phone number.

Once you receive the claim form, fill it out and sign it. Depending on the company, you may need to submit the original death certificate or a photocopy. The company might also request the original policy, but most likely not; if so, you can mark on the form that it is either lost or that you will destroy it.

Each beneficiary must complete their own set of claim forms, so be sure to make copies of the blank forms beforehand. The claim forms and death certificate must be submitted together. If there are multiple beneficiaries, they may submit their forms at the same time, using just one copy of the death certificate.

Be sure to follow the specific instructions provided, as requirements vary by company.

 If a second primary beneficiary chooses not to receive the death benefit—for example, due to receiving disability income that could be affected—they may disclaim their portion of the proceeds.

To do this:

• Write a signed letter stating the intent to disclaim the benefit.
• Include the policy number.
• Clearly state the beneficiary’s name and relationship to the deceased.
• Indicate that they voluntarily forfeit their right to the benefit.
• Include a copy of their valid ID or driver’s license with the letter. 

Once the disclaimer is accepted, the disclaimed portion will be redistributed among the remaining beneficiaries, according to the policy's terms.
