If you lost your policy and want a duplicate one you will need to write a letter or fill out a form along with a $15 or $25 check or money order (write the policy number on it) and send it to the company. If you lost your policy within a year some company may send you a duplicate for free.

You can request to receive a policy statement of insurance for free, which contains the insured and owner's name, face amount, and primary beneficiary information. It is as valid as a physical policy. (Remember that during the claim process you don't need the physical policy anymore.)
